Step 1:
All subcontractors must first prequalify prior to being awarded a project. Please be prepared with the following items to complete the prequalification process:

Information for entry

• Your Company Information

• Information on recently completed projects inclusive of values

• Three contractor references (Company Name, Contact Name, Contact Information)

• Three supplier references (Company Name, Contact Name, Contact Information)

Forms for upload

• Current Form W-9

• Contractor registration certificate

• Experience Modification Rating (EMR) worksheets from your workers' compensation policy

• Three most recent OSHA 300A Summary Logs

• Current Surety Bonding Letter (if applicable)

• Certificate of Insurances (COI) - please see insurance section on Subcontractor Terms page.
